I havent received mine but would be surprising if it doesnt appear in there
Unless I missed it, I didn't see it in the box set. :( I haven't looked through the photo book too well yet, but I am assuming it's not in there.

I ended up buying the Best Buy CD/DVD also just because I'm such a Rob Jones/White Stripes nut. :P

is that cd print version a standalone folded print that they threw inside the cd booklet that comes with the cd/dvd or does the booklet itself fold out into that print?

It's just a little 8-page CD booklet made with 2 pieces of paper, stapled in the middle and folded. This is the center spread. 9-3/8" x 4-3/4". You can see it a little better on the previous page. I'd take a photo of it to show you it better, but I carefully took the staples out and flattened it. :)

This is the Best Buy CD/DVD version with the red cover. I am not sure if the regular CD version has it or not.

Rob is sending me a metal version of this print. I should get it tomorrow. I wish.

That blackline print is beautiful kevron. Although I love both of them, I think I might like the all red one better, it's a bit more moody.
